On the day of your visit, please bring your Health History, Photo ID, insurance card, list of current medications, a personal calendar (in the event we will schedule surgery) and any questions you have for the doctor. Your questions are very important to the doctors so a written list of questions can be very helpful to ensure we answer your concerns fully.
